Adjust schema to use new primary keys (#61)

In this task I've should review indexes on the database to ensure that we are using proper indexes after changes done to add support for MAM:2.

However, after reviewing indexes and performance, we've decided that primary key on the schema needs to be changed and that requires further schema adjustments.

I've updated all database schemas. The only thing left is MongoDB but I need to think about the correct approach for it and it is not even adjusted to support stable identifiers.

I suppose all is working OK now as we are using this schema for quite a while.
