Works very well and I like it, however, there are 2 elements different from the spec above:

You used "Alternative hostname" instead of "Server address". I think "Alternative hostname" might be confusing. Why 2 hostnames? "Server address" is still not perfectly clear for many users but at least we do not have 2 hostnames. iChat uses simply "Server" in this case.

You dropped word "Optional" from a few fields. I think it might be useful to clearly indicate to the user that some fields are optional and he does not have to fill them.

The main motivation behind this task is to expose and make prominent domains on our server and make them available by default to the user. The secondary objective is to simplify the screen a bit. Please mind, the fact that some/most of the users might be not so familiar with the XMPP details. Below is a list of fields in the given order.
Current field: User JabberID replace with User name
Next is the Server name field with drop-down list with following items: sure.im, jabber.me, tigase.im, other...
Next is the Password
Next is the Confirm password
Next is Client description (resource) with default value: Tigase Messenger
Next is Other server name
Next we can have optionally some kind of separator if possible - thin line or something
Next is Optional server address
Next is Optional nickname
Next is Optional e-mail address
